ESSENTIAL FORMS FOR TEAM TRIALS
To attend a district trial or invitation regional trial students will need to have completed the following three forms. The forms must be completed and handed to the district manager/coach before the trial begins even if a parent or guardian is attending the trial.

PERMISSION TO ATTEND TRIALS - AVAILABILITY FOR SELECTION (This form includes the principal's consent (signature) and must carry the school stamp. It is available through the school and is no longer downloadable.
These forms will stay with the district manager or coach for the length of the Regional trial/competition.
If the student goes on to Met North regional selection, the Met North Sports Region will require the student to complete a similar set of forms. Advice given to the Met North Sport Office requires them to request students selected for a regional team to complete a new set of forms.
SWIMMING, CROSS COUNTRY, TRACK AND FIELD TEAMS
Students who represent the district in
swimming
cross country
track and field
will only be required to complete forms A and B which will then be handed to the team manager/s at the Met North competition.
