APPRAISEMENT (What do I do after the assessment?)

The second phase of the appraisement process is to write a support plan for each appraised student. The two items below can assist you in the writing and recording of information.

Suggested guide for writing support plan
A ‘Support Plan Guide’ (MS Word) has been developed to assist teachers in the writing of this document. The headings for the table are ‘Where you’re at’, ‘Ask yourself’, ‘So do this’.

Appraisement summary
Attached is a pro forma appraisement summary (MS Word) to assist you in the recording and organization of information gathered during the assessment phase of the appraisement process to highlight the results and types of support the students listed will require. It also assists teachers to tabulate other information relevant to the appraised students, that is ‘Teacher meetings’, ‘Support plans’, and ‘Review dates’.
